The platform includes an AI-powered care navigation software (CareGPT) integrated with the MIND at Home models to generate personalized care plans, digital assessments, 24/7 caregiver support hotlines, care team collaboration tools, referral and service coordination, medication management reconciliation, and caregiver education resources.
